Data Protection Subject Access Requests
If you are requesting information that you believe West Yorkshire Fire and Rescue Service holds about you, you are entitled to do this under the General Data Protection Regulation. Such requests are known as Subject Access Requests and can be made using the form below.

Your Subject Access Rights
Subject to certain exemptions, you have a right to be informed whether West Yorkshire Fire and Rescue Service holds any information about you (your ‘personal data’) and a right to be provided with a copy of that personal data within a one month period.
If you wish to exercise these rights please complete the Subject Access Request Form carefully, providing as much detail as possible to narrow the search and locate the precise information you require. You will be required to provide proof of identity with the form which should be sent to the address listed below.

Proof of Identity
To ensure that personal data does not fall into the wrong hands West Yorkshire Fire and Rescue Service needs to be satisfied with the identity of the requester. Consequently, you are required to provide evidence of your identity and address by supplying copies of at least two official documents which between them provide sufficient information to prove your name, date of birth, current address and signature. We reserve the right to request original documentation in some cases.
Returning the Form
Once we have received your completed application and we are satisfied that the request is valid, the process to provide you with the information will begin and may take up to one month.
